A pain at the moment of penetration. A deep ache that surfaces only with deep thrusting. Pain that begins during sex and lingers afterward. The shared feature is that pain, not pleasure, becomes the defining experience of the sexual act, and over time the anticipation of pain reshapes the relationship around it.

Dyspareunia (English: dyspareunia, painful intercourse; Japanese: 性交疼痛症, seikō-tōtsūshō) is a sexual dysfunction characterised by persistent or recurrent pain occurring before, during, or after sexual activity (particularly penetrative intercourse). It is overwhelmingly more common in women but also occurs in men. In DSM-5, it was merged with vaginismus into a single category, Genito-Pelvic Pain/Penetration Disorder, though the term dyspareunia remains in continuous use in gynaecology and sexual medicine.

Classification

Superficial (vulvar) dyspareunia: pain localised to the surface, around the vaginal opening, vulva, or clitoral region. The principal underlying conditions are vulvodynia, vestibulitis, and clitoral inflammation. Excessive pain response to stimulation (allodynia) is frequently present.

Deep dyspareunia: pain in the lower abdomen, deep pelvis, or lower back arising with deep penetration. Endometriosis, ovarian cysts, uterine fibroids, pelvic inflammatory disease, and pelvic adhesions are the principal causes. Endometriosis is one of the most common causes of deep dyspareunia.

Principal causes in women

On the hormonal axis, the low-oestrogen state of the postmenopausal period or of postpartum lactation thins and dries the vaginal mucosa (atrophic vaginitis), which becomes a major cause of friction-related pain. Long-term oral contraceptive use has also been reported to reduce oestrogen sensitivity of the vulvar and vaginal mucosa.

Among infections, vaginal candidiasis, bacterial vaginosis, herpes, and chlamydia can all cause pain. Vulvodynia is a treatment-resistant condition with a strong neuropathic-pain component, in which chronic vulvar burning and pain persist without evident infection or inflammation.

After childbirth, scarring from episiotomy or tearing, or pelvic-floor muscle dysfunction, readily becomes a cause of pain. When such pain persists beyond six months postpartum, dedicated care is warranted.

Painful intercourse in men

Sexual pain is uncommon in men, but when it occurs the principal causes include chronic prostatitis, phimosis, Peyronie’s disease (penile curvature producing distorted penetration), and epididymitis. Pain at ejaculation is often associated with prostatitis or seminal vesiculitis.

Psychological dimension

Pain is not determined by physical cause alone. The experience of pain forms a vicious cycle of “fear, avoidance, muscle tension, and further pain”, in which psychological factors maintain and amplify the condition. When sexual pain becomes chronic, sexual avoidance, reduced desire, and deteriorating partner relationships readily follow, and psychological distress itself is assessed as part of the symptom picture.

Barriers to care

The notion that “a little pain during sex should just be endured”, together with shame that makes the topic difficult to raise, deters people from seeking help. Even within gynaecology, relatively few clinicians can assess and treat sexual pain expertly, and many women report being sent home with a verdict of “nothing wrong”. Dedicated clinical communities have begun to address this unevenness in care.

Treatment

Treating the underlying condition is the foundation. Principal approaches include topical oestrogen therapy (for atrophic vaginitis after menopause or lactation), antifungal or antibacterial treatment (for infections), tricyclic antidepressants and neuropathic-pain agents such as pregabalin (for vulvodynia), pelvic-floor physical therapy, and cognitive behavioural therapy. Where endometriosis underlies the pain, separate medical or surgical treatment is required.
