Sexual addiction (Japanese: 性依存, sei izon) is the concept of a state in which sexual behaviour, the consumption of sexual content, and the pursuit of sexual stimulation progress to the point of being hard to self-control, producing real harm to one’s life, work, and relationships. It is also called “sex addiction,” “sexual compulsion,” and “compulsive sexual behaviour.”

Definition and diagnostic status

The medical diagnostic criteria for sexual addiction are not settled internationally. The DSM-5 (American Psychiatric Association) considered including “hypersexual disorder” but ultimately excluded it. The ICD-11 (WHO) includes “Compulsive Sexual Behaviour Disorder” (CSBD) among its impulse-control disorders, and it is now widely referenced internationally.

The ICD-11 criteria include a repetitive, intense preoccupation with sexual urges and thoughts, an inability to control the behaviour, distress or agitation when the urge is unmet, and repetitive sexual behaviour impairing other areas of life such as work, family, and health management. Both classifications are careful to distinguish a high but non-pathological sex drive from a clinically significant disorder.

Symptoms and behaviour patterns

Clinically reported patterns include repeated sexual acts or masturbation taking up the major part of one’s time; pornography consumption that only increases, with formerly arousing material no longer satisfying (tolerance); repeated failed attempts to stop; sacrificing work, family, and intimate relationships for sexual behaviour; and, in some cases, an inability to control urges that leads to legally or ethically problematic acts.

Mechanism of dependence

As a neuroscientific basis, sexual behaviour is noted to strongly activate the dopamine reward system. By analogy with substance addiction, a “pornography addiction model” has been proposed in which repeated stimulation alters reward-system sensitivity, so that the same stimulus yields less reward and a cycle of seeking “stronger stimulation” forms. Whether this model truly shares a neurobiological mechanism with substance addiction is not agreed in the research, and there is criticism of over-diagnosis.

Treatment and support

Cognitive-behavioural therapy (CBT) is the main treatment approach, aiming at recognising and modifying distorted sexual thought patterns, avoiding trigger situations, and acquiring emotion-regulation skills. Twelve-step programmes such as Sex Addicts Anonymous (SAA) are widespread in the West as peer-support settings, and comparable groups have appeared in Japan in recent years.

Cultural problems

The diagnosis draws criticism for the risk of pathologising a simply higher-than-average sex drive, and for over-diagnosis driven by cultural or religious norms. Research on self-reported “pornography addiction” rooted in religious values suggests that moral conflict and guilt, rather than actual usage levels, amplify the reported symptoms, a point known as the “moral incongruence” hypothesis.
